Patient Forms and What to Bring
Your forms are provided once your appointment is confirmed. This page covers what to have with you, what you will be asked to fill in, and who to call with a question.
Nothing to download, nothing to print.
There is no packet on this page to hunt for, and that is deliberate. Your new-patient paperwork is prepared for you and provided when your appointment is confirmed, because part of it depends on which clinic you are coming to, what your plan requires, and what you are coming in for.
So the only job this page has is to tell you what to bring, what you will be asked to complete, and when. Read it once and your first visit runs on time instead of running over.
If anything here is unclear, call the office at (631) 596-5096 before you come in. The front desk would much rather answer it now than sort it out while you are standing at the counter with your coat on.
What to have with you at the first visit.
Photo ID
A driver’s license or any government-issued photo identification. It gets checked once, at the start, and then never again.
Your insurance card
The physical card if you have it, front and back. Bring it even if we already ran a benefits check for you, so the front desk can confirm the numbers on file.
A list of your medications
Names and doses, including anything you take for pain. It changes how your therapist reads your response to treatment and how hard they push on the first day.
Imaging and surgical notes
X-ray, MRI or ultrasound reports, and the operative note or protocol if you have had surgery. We follow your surgeon’s protocol as written, so those pages matter.
Clothes you can move in
Loose, comfortable clothing that does not restrict circulation, plus sneakers. You will be asked to move, and denim decides the outcome more often than people expect.
Your questions, written down
The ones that are obvious in the car and gone by the time you are on the table. Bring them. The evaluation is the right place for every one of them.
Who to call, and what they can sort out.
One number covers both clinics: (631) 596-5096. The front desk handles scheduling, your paperwork and anything to do with your insurance file, and they will get a therapist on the phone if the question is clinical. You can also email support@greinerphysicaltherapy.com.
If your question is about coverage or cost, ask for a benefits check. We are in network with Medicare and Medicare Advantage. We participate with most other plans as an out of network provider, and we submit claims on your behalf. Medicaid is not accepted.
And you do not need a referral in hand to make the appointment. New York direct access lets you see a licensed physical therapist without a doctor’s referral for up to 30 days or 10 visits, whichever comes first.
