Read bioIn 2015 Seth Greiner stopped waiting for outpatient physical therapy to change and started driving to patients instead. One person, one hour, in their own kitchen or garage. Eleven years and two Hamptons clinics later, the practice has grown without ever hiring an aide.

Seth Greiner graduated from Boston University in 2005 with a bachelor’s and a master’s, cum laude, and picked up his personal training certification through the National Academy of Sports Medicine while he was still there. Then he spent roughly a decade working hospitals, schools and outpatient clinics across the tri-state area.
He learned a great deal in those rooms, including what he did not want to build. Three or four patients on the floor at once. An aide running the exercise program. A thirty minute slot that becomes ten minutes of real hands-on care once the documentation is accounted for. Nobody in those buildings was careless. The structure simply made undivided attention impossible.
So in 2015 he left and launched a mobile practice, delivering physical therapy directly to patients and clients across the Hamptons. One patient at a time, in their own environment, for the full session. In 2021 that became a clinic in Speonk. Quogue followed. The rule survived contact with both buildings: no assistants, no aides, just you and a licensed physical therapist.
Seth lives in East Quogue with his wife and their two daughters. He is a Steelers, Penguins and Mets fan, he trains with high intensity interval work and yoga, he plays tennis, and he is the head coach of Rock Steady Boxing Hampton Bays, a noncontact boxing class for people with Parkinson’s disease.

That sentence is on the front page of this practice for a reason. Two people walk in with the same MRI report and the same diagnosis, and the thing that is actually driving their pain is not the same thing. One of them sits at a desk and has not loaded a hip in fifteen years. The other plays four rounds of golf a week. Handing them the same printout is not a treatment plan, it is a filing system.
No assistants and no aides. The licensed physical therapist who evaluates you is the one who treats you, every visit, for the whole visit. It is the least scalable way to run a practice and it is the entire point of this one.
Pain, Prime, Prevent, Perform. You always know which phase you are in and what has to be true before you move to the next one. The goal of the fourth phase is that you stop needing the first three.
Every course of care starts with your story, because the history is usually where the answer is hiding. What you were doing, what changed, what you have already tried, and what you are actually trying to get back to.
Seven licensed physical therapists treat patients here, six of whom hold a doctorate in physical therapy, plus an office manager and two patient care coordinators who handle the scheduling and the insurance so you do not have to.
