Request an Appointment

Two Hamptons clinics, no referral needed in New York, and a call back within one business day. Tell us what is going on and we will find you a time.

You have probably been putting this off for a while.

Most people who fill out this form have been managing something for months. You rested it. You stretched it. Maybe you already did physical therapy somewhere and spent most of it with an aide while your therapist worked the rest of the floor. Then you stopped, because nothing actually changed.

What you are booking here is different in a way that is easy to verify: a full one-on-one evaluation with a licensed physical therapist who stays in the room with you, start to finish. No assistants, no aides, and no handing you off once the interesting part is over.

Nothing is charged and nothing is committed by asking. Fill out the form, or call the office, and a person picks it up from there.

What the first appointment actually looks like.

One session, one therapist, and a plan you leave with.

You tell the whole story

What hurts, when it started, what you have already tried, and what you want to get back to. This is the part most clinics move past in four minutes. Your therapist does not.

A hands-on evaluation

Your therapist tests how you move, finds where it fails and works out what has been compensating for it. You hear what they found, in plain language, while you are still in the room.

Treatment starts the same day

You do not wait a week for a plan to arrive in the mail. Hands-on work and your first exercises begin at the evaluation, and you leave knowing what the next few weeks are meant to look like.

Your home program goes with you

The exercises you were shown land in our home exercise app before you leave, so you can see exactly how each one is supposed to be performed.

Four things people ask before they book.

No referral needed

New York direct access lets you see a licensed physical therapist without a doctor’s referral for up to 30 days or 10 visits, whichever comes first. You can start this week without waiting on anybody else’s calendar.

The insurance question, up front

We are in network with Medicare and Medicare Advantage. We participate with most other plans as an out of network provider, and we submit claims on your behalf. Medicaid is not accepted. Ask for a benefits check and you will have an estimated cost before your first visit.

Same-week appointments, usually

Between two locations, early mornings and evenings that run past seven, there is normally something open this week. Tell us the window that works and we will try to fit it.

Speonk or Quogue, your choice

Speonk is the original clinic, with the larger gym floor. Quogue is the newer office on Jessup Avenue. Same model and same team, whichever one is closer to you.

Your appointment starts when it says it does.

"An excellent P/T with very attentive staff and therapists, and your scheduled apt start on time."

After you hit submit.

Someone from the Speonk or Quogue office calls you back within one business day. They confirm what is going on, answer the insurance question honestly, and offer you the first times available at the location you picked.

If you asked for a benefits check, that call includes your estimated cost for services. If you would rather not wait for a callback, the number is (631) 596-5096 and it reaches both clinics.
