What this website collects, why, who sees it, and how to reach a person about it. Effective August 2026.
What this policy covers.
This policy explains what Greiner Physical Therapy collects through this website, why we collect it, who it is shared with, and how long it is kept. It applies to greinerphysicaltherapy.com and to the forms published on it.
It is not our Notice of Privacy Practices. Protected health information created once you are a patient of this practice is handled in line with our obligations under HIPAA, and your Notice of Privacy Practices is provided to you at your first visit. This policy covers the website and is not a substitute for that notice. Where the two overlap, the notice is what governs your health record.
Effective date: August 2026. If this policy changes materially, we will update that date and post the revised version on this page.
The information you send us on purpose.
There are two kinds, and they are very different. The first is what you type into a form and send to us deliberately.
And the part that arrives on its own.
Where you have asked us to verify your insurance benefits, we will also collect plan details such as your carrier, member ID, group number and date of birth. That is collected because you asked for a benefits check, and you can always give it to us over the phone instead of through a form.
The second kind is standard technical and analytics data that any website receives: your IP address, browser and device type, which pages you visited, how you arrived, and how long you stayed. We look at it in aggregate to understand which pages are useful and which are not. We do not attempt to identify you from it, and we do not use this site to build advertising profiles of individual visitors.
Please do not send detailed medical history through a website form. Tell us enough to route your request and save the rest for your appointment or a phone call, where it belongs in your clinical record rather than in an inbox.
Why we ask for any of it.
Form submissions are used to contact you about care and scheduling. That means calling or emailing you back, answering your question, checking your insurance benefits when you have asked us to, and booking, confirming or moving an appointment.
If you become a patient, the relevant information moves into the practice’s own scheduling and clinical records systems, and from that point it is handled under our HIPAA obligations rather than under this website policy.
We do not sell your information. We do not rent it, trade it, or hand it to anybody who wants to market something to you.
Who else sees it.
Nobody, other than the service providers we need in order to operate. That is the company that hosts this website, the form and email services that deliver your submission to the office, our analytics provider, and the practice’s own scheduling and medical records systems. Each is given only what its function requires and is not permitted to use your information for its own purposes.
There are two narrow exceptions. We may disclose information where the law requires it, such as a valid subpoena or court order. And if the practice were ever sold or merged, contact and patient records would transfer as part of that transaction, and this policy would continue to apply to them until a published successor replaced it.
How long any of it is kept.
Website enquiries that never turn into an appointment are kept only as long as they are useful for following up with you, and are then deleted from our systems.
Patient records are a different matter. They are clinical records, and they are retained for the period New York law requires of a physical therapy practice, then disposed of securely.
Aggregate analytics data, which does not identify you, is retained inside our analytics account according to that provider’s own retention settings.
Cookies, and how to refuse them.
This site uses cookies and similar browser storage for two purposes: to make the site function as you move through it, and to collect the aggregate analytics described above. They do not store your medical information.
Every browser lets you refuse or delete cookies, and most can be set to warn you before one is accepted. Some parts of the site may behave differently if you turn them off. Your browser may also offer a Do Not Track or global privacy control signal, which we honour where the technology supports it.
The things on this site we did not build.
Some pages embed content served by other companies. When a page containing one of them loads, that company can receive your IP address and set its own cookies, under its own privacy policy rather than this one.
What that means for you.
We do not control what those companies collect and we will not pretend to speak for their practices. If you would rather avoid them, you can reach us using the phone number, email address and street addresses printed in the footer of every page on this site.
Links out to other websites work the same way. Once you follow one, you are on somebody else’s property and under somebody else’s policy.
What you can ask us to do.
You can ask what information we hold about you, ask us to correct it, ask us to delete a website enquiry, and ask us to stop contacting you. Say so by phone or email and we will act on it.
Any marketing email you receive from us carries an unsubscribe link in every message. Messages about an appointment you have already booked are not marketing, and the way to stop those is to tell the office.
As a patient you also hold rights over the health record itself, including access to it, amendment of it, and an accounting of certain disclosures. Those rights are set out in the Notice of Privacy Practices you are given at your first visit, and they are broader than anything described on this page.
Two more things, briefly.
We use commercially reasonable safeguards to protect what you send us, including encrypted transmission of form submissions and restricted access to the systems that store them. No website or method of electronic storage can honestly be called absolutely secure, so we will not tell you ours is. What we will do is ask for as little as possible and limit who can see it.
This website is not directed at children, and we do not knowingly collect information through it from anyone under the age of 13. Care for a minor patient is arranged by a parent or guardian and documented in the clinical record under our HIPAA obligations, not here. If we learn that a child has submitted information through this site, we will delete it.
