Pain is an alarm, and alarms can get miscalibrated. When something hurts for months past the point the tissue should have healed, the nervous system carrying that signal gets better at carrying it. The threshold drops. Movements that never used to hurt start to. It becomes possible to be in real, physical, measurable pain without a fresh injury anywhere, which is the part nobody explains and the part that makes people feel they are not believed.

Around that, a physical decline sets in that is entirely real. You stop the movements that hurt, so you lose range. You do less, so you lose strength and conditioning. How we treat pain that has been there a long time.

The first visit is a long conversation and a thorough examination, in that order. We want the whole history: what started it, what has been tried, what made things worse, what you have given up, and what you would do first if the pain dropped by half. Then we test movement, strength, mobility and tolerance so we know exactly where the ceiling currently is.

The plan usually pairs hands-on work with graded loading. Manual therapy and massage therapy reduce the guarding and give you a window of easier movement. Inside that window we introduce movement you can tolerate, then add to it in doses small enough that your system does not sound the alarm. Strength and conditioning follow, because a body with more capacity has a higher pain threshold, and pacing strategies come with it so the good weeks stop ending in a crash.

You get the same therapist, for the full session, every visit. That matters more here than anywhere else: a persistent pain problem takes someone who remembers what you did last Tuesday and how you felt on Thursday, not whoever is free.

We establish what you can currently do without a flare, use hands-on work to reduce guarding, and get sleep and daily routine stabilised. Nothing gets pushed before we know where your line is.

Prime: move the ceiling, one small step at a time

Graded exposure to the movements you have been avoiding, plus strength and conditioning added in doses your system tolerates. Progress here looks like a slightly longer walk each week, not a breakthrough.

Prevent and perform: get your life bigger than the pain

Pacing, flare planning, and building back the specific activities you named on day one, whether that is eighteen holes, a garden, or a full night of sleep. You leave able to run the plan without us.

01 / Can chronic pain actually go away?
The underlying condition may not be curable, and we will not pretend otherwise. Substantial reduction in pain and a large gain in what you can do are realistic and common, including for people who have had pain for years. The goal we set with you is measured in what you can do, not only in what the pain score reads.
02 / Do I need a referral, and will insurance help?
No referral is needed in New York for up to 30 days or 10 visits, whichever comes first. We are in network with Medicare and Medicare Advantage, and out of network with most other plans, and we submit claims on your behalf so the paperwork is not one more thing on your list. Medicaid is not accepted.
03 / I have already done physical therapy and it did not work. Why would this be different?
Ask what your previous course actually was. If you were seen for ten minutes and passed to an aide with a sheet of exercises, that was not really a plan. Here you get a full session with the same licensed physical therapist every visit, a dose that respects your current tolerance, and a plan that changes when your response tells us to change it.
04 / How long before I notice anything?
Most people feel some change in how a movement goes within the first few visits, because the hands-on work and the range work act quickly. Durable change in tolerance takes longer, generally two to three visits a week for at least four to six weeks, and we re-test rather than guess.
